Why singing strains the voice

Have you ever been to a church service or society event where you had to sing from an old songbook and had a lot of trouble with your voice? I know I have, often there's a sense of strain on the vocal chords and it's painful. Classical musicians and vocal artists used to have their instruments tuned much lower (C=256), matched to the tonal range of the human voice, but years ago at a music competition (I believe it was) a band from Russia decided to tune the instruments higher so that their sound would be more brilliant. The group won the competition and musical instruments and the scale itself were raised in pitch, unfortunately without regard to its effect on the human voice.
